The failure modes maintenance crews know too well
Most maintenance harm comes from a handful of repeating patterns: an isolation that missed a second energy source, a job resumed after a break without re-proving the state, a 'temporary' guard or blank that quietly became permanent, or a body in the line of a fitting under load. The scenarios here are built around exactly those patterns so the conversation lands where the risk actually is.
A prompt for verification, not just awareness
The discussion questions lean hard on verification — how will you prove the energy is at zero, how do you know the isolation is still holding, who lays eyes on the exclusion before the start. That is deliberate: 'be careful' has never isolated anything. Every brief closes by asking the crew to agree one concrete action or check before they pick up the tools.

- Does it replace our isolation procedure?
- No. It is a discussion starter. Always follow your site's isolation, permit and lockout procedures for the actual task.
- Is any of this a real incident?
- No. Every scenario is fictional and labelled as a training scenario, modelled on common hazard patterns rather than any real event.
