Hazards
Rate each hazard's likelihood and consequence from 1 (lowest) to 5 (highest). Runs in your browser.

| Hazard | L | C | Score | Level |

| Trip / slip on site | 2 | 2 | 4 | Low |
Score = likelihood × consequence (1–25). Bands: 1–4 Low, 5–9 Moderate, 10–14 High, 15+ Extreme. A guide for study — follow your employer's risk matrix and SWMS on a real job.
How to use this calculator
- List each hazard for the survey job (traffic, terrain, weather, plant, isolation, etc.).
- Rate its likelihood and consequence from 1 (low) to 5 (high).
- Read the risk score and level, and the count of hazards in each band.
How it works
The risk score is likelihood × consequence (1–25) — the classic 5×5 matrix.
Bands: 1–4 Low, 5–9 Moderate, 10–14 High, 15+ Extreme. The higher the band, the more controls you need before work starts.
Worked example
Working near live traffic. Likelihood 4 × consequence 5 = 20 → Extreme. That hazard needs strong controls (traffic management, hi-vis, spotter) before the job goes ahead.
Frequently asked questions
Is this an official risk assessment?
No — it's a study aid for understanding a risk matrix. On a real job, use your employer's risk matrix, SWMS and WHS process, which may band scores differently.
Is this official course material?
No. It is free study support mapped to surveying course levels — not official North Metropolitan TAFE content or advice. Always follow your lecturer and the official assessment brief, and check your own working.
